Plastic Surgery for Men

Text, photos, and videos by Dr. Forley

Men have embraced plastic surgery as a way to keep looking their best. A desire for self-improvement that is driven by competition in the workplace and the prevailing youth culture has resulted in men becoming an increasingly common presence in the plastic surgeon’s office. From treating facial aging to improving body contours the many surgical and nonsurgical options now available have attracted an increasingly diverse group of men in all age groups.

Men are seeking out procedures such as neuromodulators like Botox® Cosmetic to treat wrinkles, injectable fillers to restore volume, and energy devices to tighten, lift, and smooth the skin. Surgical procedures that make them look younger, more rested, and more representative of how they actually feel include the treatment of baggy eyelids, nasal appearance, excess neck, hip, and abdominal fat, and gynecomastia or male breast enlargement. The minimal downtime procedures performed today make their acceptance amongst busy professional men and women very high. A common comment that is heard from many men is “make me look better but don’t make it look obvious.” Dr. Forley’s approach, in over 25 years of practice, has always been to keep it natural and nonsurgical looking for both men and women.

Fortunately, technology has advanced to the point that we are now able to offer multiple options that enable us to find the perfect fit for every patient. Making the correct diagnosis and offering the appropriate treatment is as important in plastic surgery as in any other medical specialty. With training at some of New York’s most prestigious hospitals, Dr. Forley will devote as much time as needed during your initial office visit to determine treatment choices that will best address the issues that are of greatest concern to you.

BOTOX® Cosmetic

The repetitive contraction of muscles of facial expression and changes in skin quality over time contributes significantly to the early aging changes seen in men. Vertical frown lines, forehead lines, and crow’s feet or smile lines can be successfully treated with injectable neuromodulators, such as Botox® Cosmetic, Dysport®, and Xeomin® This no downtime, fast-acting treatment blocks the nerve impulses to the wrinkle producing muscles. Without repetitive reinforcement from the underlying muscles, skin lines and wrinkles gradually smooth out creating a more youthful and rested appearance without any telltale signs of what was done.

Injectable Fillers

Facial wrinkles and deepening of folds in the face are frequent complaints of men as they age. Injectable fillers present an affordable, effective, and long-lasting non-surgical treatment with little to no time away from work or social activities. The natural-appearing results that are achieved with injectable fillers are especially appealing to those who desire no evidence that anything was done. Etched facial lines, skin folds near the nose and mouth, loss of jawline definition and cheek hollowing can all be enhanced with this simple in-office procedure.

Lasers and Light Devices

Energy devices are utilized to treat wrinkles, face and body discoloration, facial capillaries and redness, unwanted face and body hair, and scars. The technologically advanced Nordlys system is used to deliver laser and broadband light energy treatments. Fractora and Morpheus8 combine microneedling and radiofrequency energy to improve wrinkles, skin tone, and surface texture.

Surgery is not for everyone and a non-surgical skin tightening approach may be preferred. FaceTite and AccuTite utilize bipolar radiofrequency energy to lift sagging tissues and enhance skin quality. BodyTite combines fat contouring with non-excisional skin tightening. Ultherapy uses focused ultrasound energy to lift and tone skin. Facial folds and jawline definition can be improved with Silhouette Instalift suspension sutures.

Surgical Procedures: Eyes

Loose, sagging, and bulging tissues of the eyelids can make the eyes appear smaller and lead others to ask if you are tired or stressed. In some men, a low-lying brow bone accentuates the appearance of heaviness and skin excess that develops over time. Blepharoplasty or eyelid surgery treats the excess loose skin and fatty tissue that produces sagging and heaviness of the upper or lower eyelids. In some cases, the fat is used to fill the hollow or deep circle that develops at the junction of the eyelid and cheek as we age. The eyes will appear larger, less tired, more open, and brighter following the surgical procedure. Blepharoplasty is one of the most common cosmetic procedures for men in NYC.

Surgical Procedures: Nose

The appearance of the nose that is of concern to men most often includes a bump on the nasal bridge, a prominent or broad nasal tip, or a crooked nasal appearance after sports-related trauma. Natural appearing results are achieved by improving the problem areas while preserving an aesthetic balance with the rest of your face. 3-D imaging is used by Dr. Forley to help you visualize the potential outcome of the surgery.

Surgical Procedures: Body

The type of physical appearance men desire can be counteracted by age, heredity, lack of exercise, and poor eating habits. Liposuction is a contouring procedure that removes unwanted bulges to achieve a more aesthetic blending of body outlines and is best combined with a program of fitness and nutrition to achieve long-lasting results. In some cases, the BodyTite™ radiofrequency energy skin tightening procedure can be utilized to treat skin laxity that may accompany the excess fat. The neck, hips, and abdomen are the areas most often targeted in men.

Surgical Procedures: Gynecomastia

Gynecomastia surgery improves the excessively large male breast by using liposuction to reduce the volume and prominence of the fatty tissue. Excess glandular tissue is removed via a small incision at the edge of the areola. Non-surgical skin tightening of the chest can be achieved with the BodyTite radiofrequency energy treatment.

01 Surgery is the only option to treat my sagging neck.

Men frequently complain about the sagging appearance of their neck as they get older. In some cases, it is due to an accumulation of fat that can be effectively treated by liposuction. This approach requires good skin quality so that it will retract following the removal of the excess fat. In the past, the only option for loose skin was a surgical neck lift. The advent of the minimally invasive NeckTite procedure has enabled both fat contouring and skin tightening without scars and with minimal downtime.

02 Surgery is the only option to treat my baggy eyelids.

Baggy appearing eyelids make us look tired and older. It is frequently one of the first signs of aging that we notice. The cause is primarily related to prominence of the fat pads within the lid. The tightness of the ligament located at the junction of the eyelid and cheek contributes to the circle that becomes progressively more apparent under the eyelid as we get older. Although surgery remains the definitive option for treatment, effective camouflage of the baggy eyelid can be performed non-surgically with conservative placement of an injectable filler to restore a smooth transition between the eyelid and cheek. This makes the fat pads appear less prominent. By masking the demarcation of the junction between the eyelid and the cheek, a more rested and youthful appearance is achieved with minimal downtime.

03 It will look obvious that I had something done.

Both surgical and non-surgical procedures can be performed in a way that achieves a natural and untreated appearance. The aesthetic sensibility, attention to detail, and conservative approach that is the hallmark of Dr. Forley’s approach assures you of the best possible outcome to fit both your goals and lifestyle.

04 If I take growth hormones I won’t need plastic surgery to look younger.

The anti-aging benefits of growth hormone supplements remains controversial. Extravagant claims and equally extravagant marketing has been a fixture of the anti-aging discussion for several years. Scientific studies have been conflicting and are yet to demonstrate a firm conclusion as to the usefulness of growth hormone supplementation for age related changes in the body. Considerable risk of adverse reactions and undesirable side effects with the use of growth hormone has also been reported.

05 I will have scars that are visible to others.

There are many cosmetic procedures today that do not involve scars. These include Botox® Cosmetic, injectable fillers, lasers, and energy based tightening procedures like FaceTite. Surgical options for self-improvement can frequently be performed with scars that are minimally apparent.

Julia Reed writes that Dr. Forley, "disarms me by asking what I might like to have done. Nothing, I say, sure that I mean it. And then he shows me before and after books of tiny little neck lifts, subtle browlifts involving tiny little incisions well hidden within the hairline. In ten years, he assures me, I’ll be thinking about it. I actually like him. And I fear he may be right."
